Utility Billing Account Change Form
This form is used to change the address to which a utility invoice is mailed to and/or add a tenant or property manager to an account.

Direct Pay Authorization Form
This form is used to request that payment for the utility bill be automatically withdrawn from a customer’s checking account. The form is filled out and signed and a voided blank check is attached.

Request for Final Billing
This form is used to request an estimated final water bill reading to be done for a property that is being sold. The form is generally completed by the title company handling the closing of the property. It is requested that the title company submit the request 72 hours prior to the date of closing.

Transfer of Ownership
This form is used to transfer ownership for a property. The form is completed by the title company during the closing of the property. It is requested that the title company submit the Transfer within 24 hours of the time of closing.
